There is one time non-refundable enrollment fee of $100 per child or $150 per family (2 or more children).
There is a family maximum of $300/month no matter how many children are enrolled. Financial assistance may be available to those in need. Please send confidential email to inquire.
Our physicians will gladly take care of their own long-term patients through college, but Sweetbay does not accept new patients into the practice who are over the age of 15.
What Does Membership Cover?
- Checkups (newborn, 2 weeks, 1, 2, 4, 6, 9, 12, 18 and 24 months, and then annually until age 22 years)
- Unlimited sick visits (acute and chronic care)
- Remote visits/virtual care
- School/camp/sports physical forms
- Management of minor lacerations (stitches, staples, glue)
- Developmental and Behavioral screenings
- Counseling and guidance (nutrition, development, sleep, behavior, parenting and more)
- Lactation consultation for newborns
- In-House Labs (Strep, Urinalysis, blood glucose)
- Evaluation and medical management of chronic pediatric conditions, such as eczema, asthma, ADHD, depression, anxiety and more
How Do I Pay for my Membership?
Membership fees are automatically charged monthly to your checking account, credit or debit card. Since your membership fee is automatically charged, you never have to worry about an unpaid bill or wonder if any insurance claim has been processed or how much you still owe.
What Services are NOT Covered with my Membership?
- Tests run at an outside lab. If you have insurance, the lab can use your insurance card for these.
- X-rays (We will order appropriate tests, communicate with radiologists and will provide accurate diagnostic codes for you to submit to your insurance company. Alternatively, we can help you explore reduced Direct Pay pricing.)
- Rapid infectious disease tests for Flu, Covid
- Medical ear piercing* (see below for more)
How Do I Cancel My Membership?
We hope you don't have to make this decision, but if you do, there is no penalty to you. Your membership will terminate 30 days from the time of notice. If you decide to return at a later time, there may be a re-enrollment fee.
Medical Ear Piercing
Medical ear piercing is a safe, clean and comfortable ear-piercing option and our goal is to make it festive, not scary. Medical ear piercing is performed by a medical professional (Dr. Galla!) in the privacy of an exam room.
Dr. Galla uses The Blomdahl system, which was developed entirely upon what is known as the “modern aseptic technique” within the health care industry. This means that all of the products and methods are safely designed to avoid the spreading of infection and exposure to nickel allergy during the piercing process. The sterile method guarantees that the piercing is hygienic and safe.
Medical ear piercing is available to members for $80 and to non-members for $100. Members should request an appointment through the app and non-members by sending us an email.
